responds to incoming and outgoing phone calls, physician communication; documents orders taken in a timely

and accurate manner; and is in frequent and timely communication with agency clinical staff related to clinical

Clinical Team Facilitator Basic Job Function Summary:

Under the supervision of the Nurse Leader, coordinates home health care services, labs; and any other equipment, supplies, external services for the agency clients as prescribed by the physician, and/or requested by the client/family, and in compliance with the state’s Nursing Practice Act, any applicable licensure/certification requirements, and the organization’s policies and procedures. Additionally responds to incoming and outgoing phone calls, physician communication; documents orders taken in a timely and accurate manner; and is in frequent and timely communication with agency clinical staff related to clinical care, and coordination of care. Some public/client relations responsibilities.

What you will do:

• Ensures all clinical/client phone calls and service needs are addressed timely and per policy, processes. • Obtains and accurately/timely documents necessary physician orders for home care services.

• Provides timely follow-up on physician communication regarding lab results, medication/service changes, or other changes in the plan of care.

• Responsible for assisting the office care managers, schedulers and direct care staff in coordinating assigned patients/visits, and adjusting assignments, when required, based on emergent agency/patient needs.

• Ensures all care coordination and scheduling tasks are performed accurately, timely and that required information/documentation/scheduling is complete and current in the EMR system.

• Works collaboratively with Scheduler, to assure visits are scheduled per plan of care, with necessary changes made to accommodate patient service needs under the supervision of the Nurse/Therapy Leader, Office Care Manager

• Triages appropriate operational/clinical calls to/from all internal and external customer contacts.

• Assists Nurse/Therapy Leader and Office Care Manager in assuring that patient, families/caregivers and direct care staff are kept informed of changes to scheduled visit(s) •

Minimum Qualifications:

• Current Licensed Practical/Vocational Nurse licensure in Georgia

• Two years or more of Home healthcare experience, or comparable experience in acute or ambulatory care.

• Must have current Driver’s license and reliable transportation to and from work site.

• Must be experienced in documenting in electronic medical records and Must be proficient in the use of computer office software.

• Must possess 1-3 years prior experience in a customer service role
