Your Specific Responsibilities:

This Senior Registered Nurse Supervisor is responsible for the HIV/AIDS Reengagement and Linkage to Care Program for Area 2A (Bay, Jackson, Holmes, Washington, Gulf, Calhoun). Reviews and approves leave, including leave without pay, sick leave and annual leave. Monitors attendance reports and reviews and approves timesheets for subordinates. Responsible for the supervision of staff Including appraisals, planning and directing their work, approving leave and having the authority for hiring and taking disciplinary action or effectively recommending such actions.

This position spends more than 50% of the time engaged in supervisory responsibilities. This includes communicating with, motivating, training, and evaluating employees in addition to planning and directing employees’ work. The incumbent has the authority to hire, transfer, suspend, lay off, recall, promote, discharge, assign, reward, or discipline subordinate employees or effectively recommend such action. The incumbent will also be required to develop performance expectations, meet with staff regularly to discuss performance, complete performance reviews, maintain vacation and work schedules, ensuring proper coverage, and complete timesheets.

Linkage to Care Specialist

Investigates communicable disease reports received by FDOH Bay County for Area 2A (Bay, Jackson, Gulf, Holmes, Washington, Calhoun). Incumbent will provide immediate linkage to HIV Care and initiation of ART at the time of HIV diagnosis and/or at the time of returning to care after a gap in services. Will assesses patient’s physiological, psychosocial, and environmental status then link clients newly diagnosed with HIV into HIV care to see an HIV Clinician, offer ART, receive counseling and create plans of care. Documents care provided in accordance with DOH guidelines. Educates patients and significant others regarding disease processes, prevention, and treatment including the risks and benefits of immediate ART. Encourages the continuum of care with every client newly diagnosed with HIV. Provides consultation and services to other health departments in Area 2A.

For persons identified as HIV/AIDS positive provides nursing interventions based on nursing assessment consistent with the quality standards as defined by Public Health Nursing practice based on theory and research and using the HRSA guidelines for quality care for HIV/AIDS patients. Provide interventions such as teaching, counseling, and administration of treatments and therapies as prescribed by the clinicians. Provides nursing treatments in the clinic for HIV positive individuals. Manages patients with HIV disease and ensures that physician’s/ARNP orders are followed. Tracks patients in HMS case management module for HIV/AIDS patient care. Works with clients to link clients to other care as recommended by physician or ARNP. Attend training as needed or required. Works with health department clinicians in a cooperative manner to ensure quality care for clients.

Provides HIV/AIDS and Hepatitis outreach to six county area as needed to include but not limited to providing HIV/AIDS testing and Hepatitis screenings and testing. Provide immunizations for Hepatitis 09 Program as protocols allow. Provide education and report on outreach efforts on a monthly basis to supervisor using required forms.

Will also reconnect with established HIV clients that have been lost to care and introduce the clients back into standard of care.

HIV/Disease Intervention Specialists

This position will serve as the team leader for the Disease Intervention Specialists and the Health Educators.

Works with health department clinicians in a cooperative manner to ensure quality care for clients.

HIV/AIDS & HCV Clinical Service PrEP Care and Follow-up:

Incumbent will assist HIV Specialist in the HIV/AIDS Clinic at FDOH Bay County, when necessary. Will also set up the Mobile Medical Unit throughout Area 2A for HIV Testing, HIV/AIDS Reengagement in care, and HIV/AIDS Clinic in approved areas. Assists in assesses patient’s physiological, psychosocial, and environmental status. Assists with diagnostic testing for both HIV and Hepatitis C. Monitors therapeutic plans Documents care provided in accordance with DOH guidelines. To evaluate and encourage and educate high risk negative individuals on PrEP (Pre-Exposure Prophylaxis). Provide PrEP clinical services and protocol according to CDC and State guidelines.

Required Knowledge, Skills, and Abilities:

Phlebotomy experience. Excellent communication and organizational skills.

Qualifications:

Valid driver’s license, Registered Nursing license.
